Applicant Information

Adult passports are available to those who are:

Schedule an Appointment

Applying

for the first time (ages 16 or older)

Renewing

a passport that is expired more than 15 years from the issue date

Replacing

a lost or stolen passport

Applicants who are 16 and 17 must be accompanied by a parent. Both the applicant and the parent must bring valid photo identification.

Applicant Documents

Proof of U.S. Citizenship (1)

Examples include:

Original or Certified copy of Birth Certificate with a raised, embellished seal
Certificate of Naturalization with an identifiable photo attached (or a previous passport)
Consular or Certificate of Birth Abroad

Proof of Identity (1)

Examples include:

Valid Driver’s License
State-issued ID
Official U.S. military ID card
Current (valid) foreign passport
Government employee ID card

If the Driver’s License or State-issued ID is outside of North Carolina or issued within the last six months, another form of photo identification is required. (e.g. Costco card, expired ID, school or work ID, etc.).

Two 2×2 photos taken within the last 6 months
- Visit Travel.State.Gov for information on Passport photo requirements

Photo service is provided at this location for $18.00, cash only

Application(s)

First-time applicants and those with a passport that is MORE THAN 15 years from the issue date:
- Application for a U.S. Passport (Form DS-11)
  - Downloading and printing the application is recommended, but paper copies are available on site.
Applicants with a passport that expired LESS THAN 15 years from the passport issue date:
- Application for a U.S. Passport Renewal (Form DS-82)
Applicants renewing lost or stolen passports:
- Application for a U.S. Passport (Form DS-11), AND
- Statement Regarding a Valid Lost or Stolen Passport Book (Form DS-64)

Applicant Fees

1.) Each applicant must pay an execution fee of $35.00 (cash only) to the Acceptance Facility.

2.) The application fees below must be paid by check or money order, made payable to “U.S. Department of State,” or cash.
