Applicants under the age of 16 and their parent(s) must be present to apply for a U.S. passport.

Proof of U.S. Citizenship

Examples include:

Original or certified copy of Birth Certificate with a raised, embellished seal
Certificate of Naturalization with an identifiable photo attached (or a previous passport)*
Consular or Certificate of Birth Abroad
Adopted minors must submit an adoption decree; foreign birth certificates must be accompanied by an English version.

*For minors renewing their 5-year passport:

Birth certificate is required for renewal
Old passport must be submitted with the new application
- A minor using a current or previous passport as proof of citizenship must also submit a photocopy of their birth certificate.

Two 2×2 photos taken within the last 6 months
- Visit Travel.State.Gov for information on Passport photo requirements

Photo service is provided at this location for $18.00, cash only

Application(s)

First-time applicants and minor renewals:
- Application for a U.S. Passport (Form DS-11)
  - Downloading and printing the application is recommended, but paper copies are available on site.
Applicants renewing lost or stolen passports:
- Application for a U.S. Passport (Form DS-11), AND
- Statement Regarding a Valid Lost or Stolen Passport Book (Form DS-64)

Parental Consent

All parents/legal guardians must be present and show valid photo identification. A consent form must be submitted from both parents, with the front and back copy of their ID.

If one parent cannot be present, a notarized Parental Consent form and a photocopy of the front and back of the absent parent’s photo I.D. are required.

If all parents are unable to be present, Section 4B of the Parental Consent form should list the name of the person appearing with the minor.

Applicant Fees

1.) Each applicant must pay an execution fee of $35.00 (cash only) to the Acceptance Facility.

2.) The application fees below must be paid by check or money order, made payable to “U.S. Department of State,” or cash.
